WHO QUALIFIES?

• Patients who choose not to use their plan due to high out of pocket costs.
• Those who are not in network with any of the plans we currently accept.
• Uninsured patients.

MONTHLY FEES

• $99 - Adult Individuals (18 years and older).
• $168 - 2 Adult Family Members
• $243 - For 3 Family Members (2 Adults 18 & older + 1 minor under 18 years of age).
• $288 - 4 Family Members (2 Adults 18 & older + 2 minor under 18 years of age).
• $65 - Minor Child (Under 18 or additional child over the 4-member family package).

WHAT SERVICES ARE COVERED?

• Annual Physical exams (Preventative Care) with labs (CBC, CMP, TSH panel, Lipid Panel, UA, Gynecological exams).
• Immunizations (Limited).
• Same Day Urgent Care provided at discounted rate.
• Telehealth Appointment.
• General Wellness Counseling.
• Chronic Disease Management(Hypertension, Diabetes, Asthma, Hypothyroidism).
• Contraceptive Visits.
